Peripheral Chemoreceptors
Peripheral chemoreceptors are sensory structures outside the brain that monitor the chemical composition of arterial blood and help regulate breathing. They are located primarily in the carotid bodies, small organs near the bifurcation of each common carotid artery, with additional peripheral chemoreceptor input arising from the aortic bodies. In humans, the carotid bodies provide the dominant peripheral chemoreceptor input for ventilatory control .1,2
Peripheral chemoreceptors respond strongly when the partial pressure of oxygen in arterial blood, or PaO₂, falls. They also respond to increased arterial carbon dioxide and increased acidity, or a fall in blood pH. Their ability to detect low arterial oxygen makes them especially important for the rapid ventilatory response to hypoxemia .1,2
Within the carotid body, specialized type I cells, also called glomus cells, act as the main sensory cells. When stimulated by low oxygen or other relevant chemical changes, these cells alter their electrical activity and release neurotransmitters onto nearby sensory nerve endings. Signals are then transmitted to respiratory control networks in the brainstem, which can adjust ventilation and other cardiorespiratory responses .1,2
Peripheral chemoreceptors are distinct from central chemoreceptors in the brainstem. Central chemoreception is particularly important for sensing changes in brain acidity associated with carbon dioxide, whereas peripheral chemoreceptors sense changes in arterial blood and provide the principal ventilatory sensing mechanism for reduced arterial oxygen .1,2
