Scapular Upward Rotation
Scapular upward rotation is an angular movement of the scapula in which the glenoid fossa turns more superiorly while the inferior angle of the scapula moves laterally and upward along the thorax. It is one of the major three-dimensional rotations of the scapula, alongside internal or external rotation and anterior or posterior tilting .1
Upward rotation is a normal and important component of arm elevation. As the humerus rises, the scapula progressively rotates upward and also typically posteriorly tilts. This movement helps orient the glenoid fossa to support elevation of the humerus and contributes to the total range available for raising the arm overhead. Scapulothoracic motion results from coordinated movement at the sternoclavicular and acromioclavicular joints rather than from a true anatomical joint between the scapula and thorax .1
The movement is produced through coordinated activity of several muscles. The serratus anterior and upper and lower portions of the trapezius are major contributors to scapular upward rotation during arm elevation. The serratus anterior also contributes to posterior tilting of the scapula, while the trapezius influences scapular motion through its actions on the scapula and clavicle .2
Scapular upward rotation should not be confused with scapular elevation. Elevation primarily describes upward translation of the scapula, as during a shoulder shrug, whereas upward rotation describes a change in its angular orientation. The two movements can occur together but represent different components of shoulder mechanics .1,2
