Type II Fiber Atrophy

Type II fiber atrophy is a reduction in the size of Type II, or fast-twitch, skeletal muscle fibers. It is typically identified microscopically in muscle tissue by measuring fiber diameter or cross-sectional area after distinguishing fiber types using myosin heavy chain expression or related histological methods .1,2

The term describes shrinkage of individual muscle fibers, not necessarily the disappearance of muscle fibers. This distinction is important because a reduction in whole-muscle size can result from atrophy of individual fibers, loss of fibers, or both. Type II fiber atrophy can also occur without a comparable reduction in Type I fiber size, producing a fiber-type-selective pattern .1,2

Age-related changes in skeletal muscle often disproportionately affect Type II fibers. A systematic review and meta-analysis of human skeletal muscle found that Type II and Type IIa fibers were smaller in older adults, while Type I fiber size was comparatively preserved. Age-related muscle loss can involve both fiber atrophy and loss of muscle fibers, and their relative contributions vary between individuals and muscles .1,2

Type II fiber atrophy is a morphological finding rather than a diagnosis by itself. It can occur in different clinical settings and may be nonspecific when seen on muscle biopsy without other defining abnormalities. Its significance therefore depends on the broader clinical and histological context .3
