Gastrointestinal tolerance refers to how well a person can consume a food, ingredient, or nutrient without experiencing troublesome digestive symptoms. In nutrition research, it is commonly assessed using reported symptoms such as bloating, flatulence, abdominal pain or cramping, nausea, reflux, rumbling, or symptoms associated with laxation. Greater tolerance generally means that a given exposure produces fewer or less severe symptoms at the amount consumed .1,2
Gastrointestinal tolerance is assessed largely from an individual's experience of symptoms, although measures such as stool frequency, stool consistency, and intestinal transit can provide additional information about gastrointestinal responses. These measures can help characterize the effects of an ingredient but are not equivalent to a person's perceived tolerance. Standardized methods for recording gastrointestinal symptoms are therefore important in controlled tolerance studies .1,2
Tolerance can depend strongly on both the substance and the dose. For example, poorly absorbed or nondigestible carbohydrates can retain water in the intestine or undergo fermentation by the gut microbiota, potentially producing gas and other changes that contribute to symptoms. Different carbohydrates can therefore produce different gastrointestinal responses .1
Gastrointestinal tolerance also varies between individuals and with testing conditions. Background diet, gastrointestinal health, medications, the amount consumed, and the form in which an ingredient is consumed can influence reported responses. The term therefore describes tolerance under specified conditions rather than a fixed threshold that is identical for everyone .1,2
